Are you currently unwell with a fever or currently experiencing COVID-19 symptoms? (new cough, loss of sense of smell and/ or taste)?
If you have any symptoms, you are currently clinically not able to have the vaccination. Please arrange to get a COVID test for yourself and your household members and call the surgery to reschedule your vaccination once you have recovered
Do you have any allergies resulting in anaphylaxis or hospital admission?
If you have an allergy/allergies that result in anaphylaxis then you are currently able to have the Oxford Astra Zeneca vaccination. If you have any questions please discuss them with your GP.
Are you pregnant?
If you are currently pregnant please discuss with your midwife / obstetrician
Have you been counselled on the vaccine purpose and side effects?
We want to make you aware that the vaccination does not give you COVID-19 and it does not give you complete immunity from COVID-19, you should still continue to follow all of the recommended guidelines until you are advised by the Government that it is safe. The vaccine reduces the chances of you becoming seriously ill if you become infected with COVID-19.
Most of the side effects are mild and are similar to any other vaccination and should last no longer than a week. These include:
- tenderness in the arm from the administration of the vaccine
- feeling tired
- a headache
- feeling achy
If you are experiencing any of these side effects treat yourself as you would with any other vaccination with painkillers like paracetamol.
If they persist or get worse including a high temperature you may have an infection.

Are you taking Warfarin or another blood thinning medication?
You are still able to have the vaccination, we will ensure that once the vaccine has been administered we will try to reduce the bruising you may experience afterwards.
Have you had a any vaccination in the last 7 days?
If you have had a vaccination, you are currently clinically not able to have the vaccination. Please call the surgery to reschedule your vaccination
Have you had a confirmed COVID-19 illness in the past four weeks?
If you have had a confirmed COVID-19 illness, you are currently clinically not able to have the vaccination. Please call the surgery to reschedule your vaccination
Are you currently participating in a COVID-19 trial?
If you are participating in a trial then you are not able to have the vaccination at this moment in time. Please call the surgery to reschedule your vaccination once the trial has ended
Do you consent to the vaccination?
If you do not wish to have the vaccination please call the surgery to let us know
