Register with your NHS GP Online

Step 1: Complete online registration form

Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Step 2: Provide proof of identity and address

Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address.

Please complete both steps to ensure you can be fully registered with us. Please allow 7 days before your registration will be complete and speak to the practice to confirm your registration. If you prefer to register in person please call the reception team.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).

Registration with the Practice

If you wish to register and you live in the practice boundary please complete the form below on a PC (some mobiles/tablets might work but we know there are problems). This is now in ‘Word’ format so you can save it and finish or submit it later.

As of May 20th 2020 we may accept patients that live outside of the above practice boundary. Please complete the Out of Area Assessment for below.

Important – Usual Errors

Incomplete Forms – Please check every question has been answered.

  • Please give your NHS number if you have one. It is in this format nnn nnn nnnn, for example 123 456 7890
  • You MUST expect to live in the practice boundary for 3 months or more (see ‘Temporary Resident’ below if you are here for less than 3 months).
  • If you have NEVER had a UK GP before, we need the date you entered the UK
  • The Previous address for all applicants is the one your previous UK GP had for you
  • If you get a message to call us about your registration it is usually because the NHS has a different address than the one you gave us, please call us urgently.

How to submit

Print it off after completion and bring it in or post it to us at the above address

What happens Next

If you have included a mobile number and consented to receiving messages we will send you a message when your application has been processed. You should then bring in identification to obtain your ONLINE SERVICES access details.
